25th Self-Help Website Forms - If you need to represent yourself, this website can assist you by outlining relevant laws, procedures and resources. For certain issues, forms are available to fill out on your computer then file with the Clerk of Court. This service in no way constitutes legal representation and is brought to you as a courtesy through the combined efforts of the 25th Judicial District Court and the Louisiana State Bar Association.
Change of Address - If your address has changed, please complete this form and submit to the Court for update and future services.
Conditions of Active Probation - This form is for use in either division of court for active probation conditions in felony matters.
Conditions of Inactive Probation - This form is for use in Division B for pleas of guilty in misdemeanor matters for defendants that are being placed on inactive probation.
Courtroom Technology & Evidence Presentation System Information - The 25th JDC has an Evidence Presentation System in both divisions of court. If you plan to use the evidence presentation system in a hearing or trial, it is recommended that you contact the division in which you will be appearing for instructions on how to use the equipment. Domestic Abuse Battery Guilty Plea in Division B - This form is used for Pleas of Guilty and Waiver of Rights to Louisiana R.S. 14>35.3 Domestic Abuse Battery in Division B.
Driving While Intoxicated Guilty Plea in Division B - This form is for use in Division B for Pleas of Guilty and Waiver of Rights in Louisiana R.S. 14:98 Operating a Vehicle While Intoxicated.
FINS Referral Form - This form can be completed online, printed and submitted to the Department of Youth & Family Services Administrator by email.
Guilty Plea Form - This electronic plea form was created to assist with felony or misdemeanor pleas of guilty taken in Division B in accordance with La. CCRP. art 562.
Notification to Sex Offender - This form is for use in either division of court for notifications to sex offenders upon guilty plea.
Notification of Supervised Release of Sex Offenders - This form is for use in either division of court for the Notification of Requirements of Supervised Release of Sex Offenders.
Possession of Marijuana Guilty Plea Division B - This form is for use in Division B for the Plea of Guilty and Waiver of Rights pursuant to Louisiana R.S. 40:966 Possession of Marijuana.
Protective Orders - Protective order forms can be obtained from the Clerk of Court’s office or by clicking HERE. Please consider the time when planning to pick up these forms and fill them out. It usually takes thirty (30) minutes to an hour to complete the forms. The case must then be assigned to a division, and a judge must sign the order. Waiver of Arraignment - Fillable Form - This form can be filled out, printed, signed and submitted to the Court to arraign clients when allowed by Rule 18.
